Panel Questions Denial of Requests for Image

Like the first shuttle disaster, this was a management failure. They refused to listen to the engineers.

The request... came from "tens" of engineers directly involved in assessing the potential damage caused when a piece of foam debris flew off the external tank and struck the orbiter's left wing 81 seconds after Columbia's Jan. 16 liftoff.
